    To complete this online form you will need:

    - Nominee's information (email, phone number, etc.)  You may only self-nominate for longevity awards.

    - Names and email addresses of people who will endorse the nomination by answering questions in an online form via a link that will be sent to them.  Awards may require 0-4 endorsements, depending on the award.  Longevity awards do not require endorsements. Endorsers will receive an email with instructions immediately after you complete this form.

    You may cancel or exit at any time but information is not saved unless you select the "save and continue later" option.  If you select this option Jotform will email you a link that you can use to complete the online form. 

    Council staff will not see your submission until you push the "submit" button below.  Once you push the "submit" button you will receive an automated confirmation email from Jotfom that includes a tracking number called a "Nomination ID".

    As you begin to fill in the form, additional questions will appear based upon your responses.  For example, if you select "longevity award" below, you will see a series of questions regarding the volunteer's longevity.

  • Please Note: If it's been 1 year and 10 months, but the award won't be presented for a couple of months, then the spirit of "approximately 2 years" has been met!

    Longevity numeral guards recognize the individual for combined years of membership both as a youth and adult member.  Numerical guards are awarded in 5 year increments. (20, 25, 30, etc)

  • These questions are NOT used to determine whether or not you are eligible for a
    Longevity Award. That is determined solely by the number of years you have been registered as a Girl Scout. This information is used to capture meaningful
    information for possible use by the Council Recognition Panel and other groups. You may write as little or as much as you want.

    - For 5-25 years, they are usually recognized at the local level (service unit or association).
    - For 30+ years, they are usually recognized at the Annual Council Recognition Event held in conjunction with the GSCNC Annual Meeting.

    To ensure that we appreciate volunteers for each individual aspect of their service, a nomination is best when it is purposely focused. Recognize someone for helping at the troop level, and then later recognize them for serving as a delegate.

    A position can be formal such as "Service Unit Manager" or informal such as "spaghetti dinner organizer".

    Please provide the information for the required number of endorsers.  Before listing someone as an endorser, please contact them to ask if they are willing and able to submit an endorsement within the desired time frame.

    If you will be one of the endorsers, include your information below so that you also receive the email with the special link.

    Please double-check the email address carefully. A bad email address will delay the award package.

    You also have the opportunity to include a brief introductory message to each
    endorser. You might use this section as a memory-jogger to the letter writer
    ("remember last week I asked you to write an endorsement for . . . "). This message will be included in the email the endorser receives.

    Most sponsors (that's you, {whatIs}) also like to write an endorsement for the nominee's ({whatIs11} {whatIs53}) award.  So we've already entered your information into "endorser #1".  If you do not wish to be an endorser you may overwrite your information with a different endorser.
